Psoriasis - An Exciting Breakthrough

There’s good news for the hundreds of thousands of people who suffer with psoriasis. Psoriasis is a skin condition characterised by scaly red blotches which commonly appear on the elbows and knees but can affect the entire body including the scalp.

Researchers have discovered a special extract of the plant Mahonia aquifolium contains potent alkaloids that are effective in the fight against Psoriasis, Eczema and related skin conditions. So, what is Mahonia aquifolium?

Research

Mahonia aquifolium has been recommended by Medical Herbalists for many years for people suffering from Psoriasis & Eczema. It has been applied topically and taken internally and both forms have been shown to be effective.

The first study to attract media attention about Mahonia aquifolium was an open, prospective multi centre trial in 89 dermatological practices in Germany, where 433 patients with subacute and chronic forms of psoriasis were treated with Mahonia aquifolium ointment.

Of the 433 patients who entered into the study, 375 were treated over the planned period of 12 weeks, or dropped out of the study early because of total clearing of the skin, A modified PASI score fell significantly from 5.5 plus or minus 4.0 to 2.3 plus or minus 2.6.

According to an overall evaluation by dermatologists, symptoms improved or disappeared in 81.1% of patients; and according to a subjective evaluation by the patients, 79.7% improved or healed completely. The tolerability of Mahonia aquifolium ointment was evaluated as good or very good by 82.4% of patients.

Significant improvement

There was a significant improvement in the quality of life, which was used as a subjective parameter throughout the study. While 30.1% of patients had significant or severe symptoms at the start, this was the case for only 5.6% after 12 weeks. M. aquifolium ointment is thus a well-tolerated preparation which should find a place in the treatment of psoriasis.

Later, in a multi-centred, randomised, controlled study, 93 patients suffering from mild to moderately severe psoriasis received treatment by either topical application of Mahonia aquifolium ointment or internal ingestion of Mahonia aquifolium extract. At the end of the treatment, significant improvement had occurred in more than 70 per cent of patients in both groups.

More recently, an article published in 2005 in the American Journal of Therapeutics detailed a further three studies all of which confirmed the beneficial effects of Mahonia aquifolium in the treatment of Psoriasis.

The first study was an open-label study to evaluate the safety of Mahonia aquifolium in 39 patients treated for 12 weeks. Assessments made were modified PASI, global assessment, psoriasis history questionnaire, Dermatology Life Quality Index, and Psoriasis Disability Index. The results revealed a statistically significant improvement in PASI score and Dermatology Life Quality Index after just four weeks of treatment.

The patients were followed up a month later and the positive outcomes were shown to have continued.

In the second study, a clinical trial involving 32 patients with mild to moderate bilateral psoriasis treated up to 6 months. One side of the body received Mahonia and the other standard psoriatic treatment (eg, Dovonex cream). The primary outcomes were patient ratings of the Mahonia-treated side alone and the comparison between treatments received on each side of their body.

The results revealed that eighty-four percent of patients rated the Mahonia-treated psoriasis as 'good to excellent' response. When compared with standard treatment, 63% of patients rated Mahonia aquifolium 'equal to or better' than the standard psoriatic treatment.

Improvement with ONE week

Study 3 was an observational study of 33 patients with mild to moderate bilateral psoriasis treated for a one month period. The results showed improvement in psoriasis after just one week of treatment. The side treated with Mahonia did as well or better than the side treated with the vehicle cream.

The report concluded that results from all three clinical trials are in agreement with published data that include placebo-controlled studies. Taken together, these clinical studies conducted by several investigators in several countries indicate that Mahonia aquifolium is a safe and effective treatment of patients with mild to moderate psoriasis.

Conclusion

Psoriasis: Treatment According To Symptoms Types

Psoriasis is a skin disease which severely affects skin and joints. Generally, patient suffering from this disease have red scaly patches over the skin, these patches are known as plaques. Excessive cell production on the psoriasis affected area is the cause of plaque generation and inflammation. Sometime the plaque also acquires slivery-white appearance. Psoriasis can be localized or it can cover whole body according to the severity of disease. Psoriasis has affected the human beings for very long time; though earlier it was believed to be a type of leprosy.

An exact cause for Psoriasis is not known. It is believed that Psoriasis affects the body because of malfunctioning of the immune-system. New Study Shows Humira Affectively Treats Psoriasis

The Biological drug Humira which is traditionally used to treat rheumatoid arthritis has recently been shown to affectively combat Psoriasis.

According to Biologicaldrugreport.com, “This is the second Phase III study to show positive results for Humira, a biologic currently approved for treatment of rheumatoid arthritis, and Abbott Laboratories said it intends to file during the first half of this year for U.S. and European regulatory approval for Humira in psoriasis. Two other biologics used to treat rheumatoid arthritis, Amgen Inc.'s Enbrel and Centocor's Remicade, have already received FDA approval for treating psoriasis.”

In the recent study, 1200 patients plagued with moderate to severe psoriasis were randomly given doses of Humira every other week or received a placebo.

The results of the study concluded that 71% of the patients that received Humira showed at least 75% improvement over normal signs of the disease after the 16 week treatment cycle.

The article also states, “Humira patients who achieved at least a 75 percent improvement at week 16 continued to receive the biologic, and at week 33, the 490 patients who maintained their 75 percent improvement were randomized to either continue receiving Humira or a placebo. At the end of one-year, 28 percent of patients receiving the placebo had seen their condition deteriorate significantly compared to 5 percent of patients who stayed on Humira, according to the researchers.”

The results showed that Humira was statistically better at treating psoriasis than the current standard methods used to treat the disease. Humira has a great chance to potentially be approved for the treatment of psoriasis.

Eczema and Psoriasis - An Inexpensive and Effective Treatment

The purpose of this article is to tell the world what I have learned from my own personal experience about an inexpensive and effective treatment for eczema. Because psoriasis is a very similar affliction, there's a good chance that this treatment would be effective for psoriasis too. Eczema is a skin irritation characterized by red, flaky skin that sometimes has cracks or tiny blisters. It's believed to be hereditary and if both parents have it, there is an 80% chance that their children will have it too.

Once upon a time, I had medical insurance coverage and I was able to afford any medication my doctor prescribed. I used to have a slew of little bottles and creams that were somewhat effective in relieving the itching, but nothing I ever tried had any effect whatsoever in terms of reducing the severity or frequency of eczema attacks. Topical applications of cortisone cream have a limited effect in terms of relieving the itching, but cortisone just suppresses eczema and can actually cause it to spread. There's a theory that eczema can be brought on by stress, and I believe that may be true. After experiencing a year long constant virulent attack during 2004 through 2005 when I was under severe stress, I decided to seek medical help in order to find out whether anything new had been discovered regarding eczema treatment. The only treatment I didn't already know about involves exposure to ultraviolet light radiation which is very expensive. For many years there have been medications that are taken internally, but all of them require regular blood testing for possible liver damage. For me, this is out of the question because any medication that is capable of causing liver damage comes under the category of unacceptable.

One day I was returning from a doctor visit when I passed a health food store, and I had some time to spend so I went in. The woman behind the counter turned out to be a certified nutritionist so I asked her whether she knew anything about eczema treatment that the doctors don't know about. She said "Yes, I do." I said "Really. Tell me." She brought out a small bottle of liquid zinc and told me that some of her customers had reported success with topical applications on their eczema sores. So I bought a bottle. I figured "What have I got to lose? Nothing. Twenty bucks, maybe." To my surprise, it relieved the itching and seemed to have some limited effect in reducing the inflammations. The health food store lady had told me that zinc is a healer so I started thinking about it. I remembered that Desitin is a very effective and well known treatment for baby rash and that the active ingredient is zinc oxide. I thought "If liquid zinc is effective topically, how much more effective could it be if I take it internally?" Being inherently cheap, I was a bit bothered by the idea of buying another small bottle of liquid zinc and I knew that zinc tablets are inexpensive. And that certainly appealed to my wallet. I discussed my idea with the nutritionist who agreed that it could work. So I began with the 75 mg. daily dose that the woman recommended and gradually worked my way upwards until I reached 200 mg. a day.

Eczema makes me so angry that I often curse "the eczema Nazis" each time the first blister occurs with the unmistakable itch that feels like it originates in my bones. I say to myself, "Damnation, I'm under attack again!" Now I say "Come on zinc, be John Wayne and kill every last one of those eczema Nazis!" Eight months after I began my inexpensive experiment with zinc taken internally, I can report real progress. Where previously one tiny eczema blister always signaled a serious outbreak, now it stops without spreading any further and heals very quickly. The indicative first blister is not necessarily accompanied by itching either. To me, this is very significant because it constitutes a completely new pattern that has never before manifested in my entire life.

If you want to try taking zinc internally, I'd recommend "chelated" zinc because zinc is a mineral and minerals are not easily absorbed into the bloodstream. Back in the early 1980's, the health food industry discovered that if minerals are chemically linked to something the body can easily absorb, they are far more effective. The chemists who invent these things decided to use amino acids which are natural proteins, and proteins are easily absorbable. The process of chemically linking a mineral to an amino acid is called chelation. I've written all this with a sincere desire to help others benefit from my experience and I really hope that somebody out there has found my article to be valuable.

It is possible to mistake Scalp Psoriasis for Seborrhoeic Dermatitis though there are differences between the two. The main way to tell Scalp Psoriasis apart from anything else is by its silver colored scales that appear on the affected skin.

How the Condition is treated

Whilst it is possible for this condition to clear up on its own, there are treatments available which can really help to control it. The type of treatment you should use depends entirely on the severity of the Scalp Psoriasis itself.

If you have a mild case of Scalp Psoriasis, some tar treatments may be best, as may Salicylic acid treatments. For more severe cases topical treatments such as creams and lotions may be prescribed, or occasionally ultraviolet light therapy may be needed. As each person is different, treatment does vary and often treatments are combined in order to be the most effective and to eliminate the condition completely. The treatment may also be changed occasionally due to the scalp becoming used to a treatment that has been used for long periods of time.

You should not expect fast results as usually the condition takes around eight weeks to show any improvement. Once the condition is under control, it is important to use products such as a tar shampoo around twice a week to stop the condition from reoccurring.

Generally tar products work really well and they are available over the counter in shampoo form, cream form, gel form and oil form. Over the counter treatments are generally low dosages and are for mild forms of the condition only. However, your doctor may be able to prescribe you a higher dose if needed. Whilst it may be really effective, a lot of people prefer not to use this type of treatment because it has a strong odor and it can also stain bedding as well as grey hair.

Tar treatments come in two forms which are coal and wood tar. Whilst coal tar is the most common treatment available, wood tar soaps are really effective in treating Scalp Psoriasis. Also, soaps are a lot cheaper than shampoos and they last longer too.

Psoriasis is a condition of the skin that is little understood. Normally skin cells take 28 to 30 days to grow to maturity, but in people who have an outbreak of psoriasis this process is shortened to 3 to 6 days. The scaling, itching and inflammation of the skin is caused by the over build-up of skin cells.

It is believed that the main cause of psoriasis is driven by a certain type of white blood cell known as a T-cell. T-cells are one of the body's defenses against disease and infection, but in the case of psoriasis they leap into action unintentionally and begin to trigger other immune systems which accelerate the creation of new skin cells. Because the skin cells grow and mature quicker then normal they also die off much faster, creating a build up of dead skin cells on the surface. This build up of dead skin cells forms the characteristic plaques, or flaky patches, that sufferers of psoriasis are familiar with. The redness and inflammation of the underlying skin is a product of the increased blood supply needed to feed the new skin cells that are growing at an accelerated rate below the plaques.

While anyone at any age can suffer from psoriasis researchers have found that people in their thirties and sixties are more likely to develop this skin condition than people in other age groups. Why this is the case no one knows. It is also apparent that heredity can be a marker for an increased chance of contracting psoriasis. Children who have one parent who suffers from psoriasis have a ten percent chance of contracting the disease. That percentage goes up dramatically to fifty percent if both parents have this condition. Scientists, who have done studies on families afflicted with psoriasis, have been able to identify genes that are known links to the condition.

The exact causes of psoriasis remains something of a mystery but there are known triggers that are linked to outbreaks. Many people who suffer from this disease notice that there are times the condition is worse and times it seems to get better. Reactions to certain drugs, such as lithium and beta blockers, have been shown to trigger a psoriasis outbreak and in extreme instances make the condition worse.

Because psoriasis is caused by a faulty immune system anything that stimulates the body's defenses can bring about an outbreak. An injury such as a cut or bruise or an infection, particularly strep, can all cause an outbreak.

Another factor that can cause an outbreak of psoriasis is stress. People who suffer from psoriasis have noticed that as their stress levels go up so does the potential for a severe outbreak of their skin inflammation. Psoriasis in and of itself also causes stress leading to a self perpetuating problem of continual outbreaks that increase in severity as a person's stress level increases.

One way that psoriasis cannot be caught is through contact with someone who suffers from the disease. It is not a contagious skin condition, and although it is unsightly and can cause repulsive reactions from those who do not suffer its affects it is important to remember that those who suffer from it need the reassurance and support of friends and family along with proper treatment to help them live a normal life.

Smokers are at High Risk for Psoriasis

Although dry skin has long been associated with smoking, not until recently has it also been linked to psoriasis. Thousands of doctors are now advising patients to use a good shielding lotion as treatment for psoriasis, but will no doubt soon be adding cessation of smoking to their recommendations.

Psoriasis is considered a chronic skin condition and presents as elevated, dry, scaly, itchy patches on the skin. The most common sites are the elbows, knees and other parts of the legs, the scalp, lower back, face, palms, and the soles of the feet. About one third of the diagnosed cases are genetic, although the first outbreak is frequently triggered by stress or physical injury. The condition is driven by the immune system – T cells, the white blood cells that help protect the body from infection and are responsible for creating scabs over wounds, become overactive and trigger other immune responses. These lead to inflammation and rapid turnover of skin cells.

The immature skin cells then rise to the surface and form the scaly patches.

A recent study, published in the December issue of Archives of Dermatology, found that those who smoke more than a pack of cigarettes per day were at twice the risk of having more severe psoriasis than those who smoke 10 cigarettes or less, and that patients who smoke are more likely to have psoriasis.

Although no causal relationship was established, lead researcher Dr. Gerald G. Krueger, a professor of dermatology at the University of Utah School of Medicine stated that if one is not genetically predisposed to psoriasis, “one of the things that you can do to make sure that you get psoriasis is smoke."

Psoriasis - Causes, Symptoms and Treatment

Psoriasis is believed to be an immune-mediated disease. Psoriasis is an inflammatory skin condition. Between 10% and 30% of people who develop psoriasis get a related form of arthritis called “psoriatic arthritis,” which causes inflammation of the joints. Females develop plaque psoriasis earlier than males. Psoriasis usually occurs in adults. It is affected around 2% of the population in the UK. It sometimes runs in families. Several factors are thought to aggravate psoriasis. These include stress and excessive alcohol consumption. Individuals with psoriasis may suffer from depression and loss of self-esteem. As such, quality of life is an important factor in evaluating the severity of the disease. There also is a genetic component associated with psoriasis. Approximately one-third of people who develop psoriasis have at least one family member with the condition.

There are five types of Psoriasis Plaque psoriasis is the most common type of psoriasis. About 80% of people who develop psoriasis have plaque psoriasis, which appears as patches of raised, reddish skin covered by silvery-white scale. The first peak occurrence of plaque psoriasis is in people aged 16-22 years. The second peak is in people aged 57-60 years. These patches, or plaques, frequently form on the elbows, knees, lower back, and scalp. However, the plaques can occur anywhere on the body. The other types are guttate psoriasis (small, red spots on the skin), pustular psoriasis (white pustules surrounded by red skin), inverse psoriasis (smooth, red lesions form in skin folds), and erythrodermic psoriasis (widespread redness, severe itching, and pain). Regardless of type, psoriasis usually causes discomfort. The skin often itches, and it may crack and bleed

Causes of Psoriasis

Treatment of Psoriasis

There are 3 basic types of treatments for psoriasis:

(1) topical therapy (drugs used on the skin), (2) phototherapy (light therapy), and (3) systemic therapy (drugs taken into the body). Vitamin D reduce skin inflammation and help prevent skin cells from reproducing. Vitamin D analogue that may be used alone to treat mild to moderate psoriasis.There can be substantial variation between individuals in the effectiveness of specific psoriasis treatments. Moisturizers help sooth affected skin and reduce the dryness which accompanies the build-up of skin on psoriatic plaques. Ointment and creams containing coal tar, dithranol (anthralin), corticosteroids, vitamin D3 analogues (for example, calcipotriol), and retinoids are routinely used. Some topical agents are used in conjunction with other therapies, especially phototherapy. Tazarotene (Tazorac) is a topical retinoid that is available as a gel or cream. Tazarotene reduces the size of the patches and the redness of the skin.

Do you, or someone you know, suffer from psoriasis skin disease? Do you know the cause? Did you know that Psoriasis may be one of the oldest recorded skin conditions? It was probably first described around 35 AD. Some evidence indicates an even earlier date. Yet, until recently, little was known about psoriasis.

Psoriasis is the most common skin condition known today. People who develop psoriasis have plaque psoriasis, which appears as patches of raised, reddish skin covered by silvery-white scale. These patches, or plaques, frequently form on the elbows, knees, lower back, and scalp. However, the plaques can occur anywhere on the body.

Psoriasis skin disease is a chronic condition because there is currently no cure. People often experience flare ups and remissions throughout their life. Controlling the signs and symptoms typically requires lifelong therapy. Psoriasis usually causes much discomfort; the skin often itches, and it may crack and bleed. In severe cases, the itching and discomfort may keep a person awake at night, and the pain can make everyday tasks difficult.

Treatment depends on the severity of the condition. Some psoriasis is so mild that the person is unaware of the condition. Recently, research has significantly advanced in the understanding of psoriasis. One important breakthrough began with the discovery that kidney-transplant recipients who had psoriasis experienced clearing when taking cyclosporine. Since cyclosporine is an immunosuppressive medication, this indicates that the immune system is involved.

The National Psoriasis Foundation reports that 56 million work hours are lost each year by those who have psoriasis. Additionally, a survey conducted by the National Psoriasis Foundation in 2002 indicates that 26% of people living with moderate to severe psoriasis have been forced to change or discontinue their normal daily activities.

Unfortunately, more than 4.5 million adults in the United States have been diagnosed with psoriasis, and approximately 150,000 new cases are diagnosed each year. An estimated 20% have moderate to severe psoriasis. While psoriasis occurs equally in males and females, more recent studies show that there may be an ethnic link.

It seems that psoriasis is most common in Caucasians and slightly less common in African Americans. Additionally, psoriasis is most common in Scandinavia and other parts of northern Europe. It appears to be far less common among Asians and is rare in Native Americans.

However, living with this lifelong condition can be physically and emotionally challenging. Itching, soreness, cracked and bleeding skin are common. Several studies have shown that people often feel frustrated. In some cases, psoriasis limits activities and makes it difficult to perform job responsibilities.

Studies also have shown that stress, anxiety, loneliness, and low self-esteem are part of daily life for people living with psoriasis skin disease. Imagine the embarrassment and pain an individual suffers with this condition.

Some Effective Natural Cure for Psoriasis

Psoriasis is a non-contagious skin disease which causes patches of thick, red skin covered with silvery scales. Itching and burning accompanied by pain are the common symptoms of psoriatic skin. The inflammations are observed mostly on the hands and legs, but they could spread to the fingernails, the toenails and even to the mouth and the genitalia.

In complicated form, psoriasis may cause pain in the joints and cracked skin. If there is an injury or a insect bite in the affected part, then there could be severe pain and aggravation, accompanied by bleeding.

Causes of psoriasis are not properly known. There may be a family prevalence of the disease, as 33% of psoriatic patients have another close family member suffering from the same disease. It may be due to viral and bacterial infections, obesity, remaining away from sunlight for long periods of time, stress, cold climate, abrasions on the skin and even alcohol consumption and smoking.

In Ayurveda, diseases such as Eka-kushta, Kitiba-kushta and Sidma-kushta are quite alike to the condition of psoriasis. These are vata vitiations. Treatment is through a very carefully prescribed dietary regimen and drug therapy.



(1) Useful Herbs in the Treatment of Psoriasis

- Black Nightshade (Solanum nigrum)

The juice of the leaves of the black nightshade can be applied locally on the affected regions. It helps in reducing the inflammation of the skin, and also has beneficial properties in allaying the pain and the itch.

- Garlic (Allium sativum)

Garlic is a blood purifier. Eating a few cloves of garlic every morning on an empty stomach filters the toxins out of the blood and provides protection against psoriasis.

- Jasmine (Jasminum polyanthum)

The flowers of the jasmine have good effects in the treatment of psoriasis. The flowers are crushed, made into a paste and are directly applied on the inflamed parts. This application reduces the pain and the itchiness of psoriatic inflammations.

- Onion (Allium cepa)

Onion has blood purifying properties. Hence, it is made an important part of the diet of a person suffering from psoriasis.

(2) Dietary Treatments for Psoriasis

A strict dietary regimen is very important in psoriasis. The wrong kind of foods can aggravate this condition, whereas the right foods can help in early treatment. The following dietary guidelines must be followed:-

- Avoid all hot, spicy and pungent foods. These take long time to digest and cause toxins in the blood, known as ama. When the ama builds up in the body, the psoriasis can spread to a great extent. Also, avoid all foods that cause constipation.

- Salts and curds must be avoided as they can inflame the skin further. This will cause greater itching and hence more discomfort. Rock salt can be taken in small quantities for flavoring, but not the salt readily available in the market.

- Cut down on fats, animal proteins and sugars. Also consume less quantity of food.

- Bitter tastes are preferable. These foods help in purifying the blood and also facilitate the kidneys to flush out the toxins from the blood. For this purpose, foods like bitter gourd, the bitter variety of drumstick and neem are very ideal.

(3) Ayurvedic Treatments for Psoriasis

The Panchkarma therapy is utilized in Ayurveda for the treatment of psoriasis. This is the first step because unless the toxins are removed from the body, it will not respond to any medication. In severe cases, purgation and bloodletting is done to cleanse the body internally. Oleation is employed using medicated ghee with bitter herbs. Commonly used herbs are neem, kutki and guduchi.

Once the purification of the body is done, Mahatikta ghrita or Panchatikta ghrita are prescribed to the person. The dosage is given in one teaspoon quantity twice a day for a month. Any guggulu preparation can be given together for better results.

There are also some popular Ayurvedic oils for external application. These are Mahamasha taila, Pinda taila, Kasisadi taila and Chandana Bala Lakshadi taila. Kushta Rakshasa taila is prescribed only for application when there is severe itching. This oil reduces the itchy sensation and prohibits the psoriasis from spreading.

(4) Home Remedies for Psoriasis

- Take one teaspoonful of sandalwood powder in three cups of water. Boil it till it is reduced to one-third the quantity, i.e. to one cup quantity. Add some rose water and sugar to it. Take this thrice a day.

- Make the powder of some almonds and boil them thoroughly in water. This is used for external application on the affected part. Keep it overnight and wash it next day with water.

- Shikakai (soap nut) boiled in water and applied on the affected parts also gives good relief.

- Attach a fresh thin banana leaf on the affected part.

- Fry thin slices of onion in ghee and eat them.

- Apply lime juice that has been diluted with water externally on the psoriatic patches.

- Cabbage soup is good for treatment of psoriasis. Drink one bowl of it everyday.

Exercise is good for psoriatic people, but care must be taken not to perform exercises like jogging and running which will put more pressure on the knees. Cycling and waling are good. Leading a sedentary life will aggravate the psoriasis further. It is also important to keep the mind stress-free through yoga. Stress can aggravate psoriasis.

Psoriasis is a dermatology inflammation (inherited) that causes cells to grow too quickly. •Biologics – This new approach, in which patients inject themselves with drugs, targets the immune system and blocks inflammation. Biologics carry fewer side effects than traditional dermatology psoriasis treatments and can be used for longer periods of time.

Over-the-counter products aren’t as effective on thick, red psoriatic patches as dermatology treatments but they still offer relief. Start with a 15-minute soak with mineral-rich bath salts, which are soothing, and cover with rich creams and ointments that help seal moisture to the skin.

Until the past 10 years, psoriasis was largely an under-researched condition. With little statistical studies done, there is not enough knowledge about it. Psoriasis and Psoriatic Arthritis

Psoriasis is a debilitating skin condition that can affect the elbows, knees, scalp, lower back, face and indeed, any part of the body., including the genital area.. It may also affect the fingernails and toenails making them thick and discoloured. The condition is evidenced by red scales and inflammation of the skin which becomes very itchy and distressing.

This occurs when the cells of the skin rise too quickly from their source below the surface of the skin before maturity. There is often a family history of psoriasis. Around thirty percent of sufferers will also experience joint inflammation and this condition is called psoriatic arthritis.

I was in my early twenties when I became aware of a reddening of my elbows and knees. At first, I took little notice but then the scaly patches and the itching began. These then spread to my knuckles, ankles and some small patches on the lower legs. No amount of moisturising lotions or the like seemed to make any difference. Finally, I went to my physician who diagnosed my condition as psoriasis.

I began a regime of applying corticosteroid lotions and creams and using coaltar solutions to the affected parts of my body but even these appeared to have little effect.

At times, I experienced significant physical discomfort. Itching and pain interfered with basic functions, such as sleep. This in turn left me feeling constantly tired and unable to cope with day-to-day chores, thus putting a strain on my family relationships. I began to feel self-conscious about my appearance and had a poor self-image that stemmed from fear of public rejection. Psychological distresses in turn lead to significant depression and social isolation.

Sometimes the condition got worse and then improved so I was then able to identify a number of things that produced flare-ups including infections, stress, and climate changes. There were also certain medications that seemed to trigger outbreaks or worsen existing patches.

The years rolled by and the disease continued to progress until I had large patches of psoriasis on my knuckles, elbows, finger joints, knees, lower legs, feet, and even the webbing of my toes. I also had it in my scalp and covering a large portion of my back. I lived in misery with the constant itching and discomfort.

Around six months ago, I received a phone call from my mother telling me she had just been diagnosed with psoriatic arthritis. Because this condition is genetic and I already had the skin condition, it was important that I see a rheumatologist to be tested. Of course, it was no surprise when I found that I too had psoriatic arthritis.

Blood tests conducted for this condition may rule out other conditions such as gout or rheumatoid arthritis. There is a particular antibody normally present in rheumatoid arthritis that is not present in psoriatic arthritis. These tests will often also show elevated erythrocyte sedimentation rate or ESR which measures inflammation. They often show mild anaemia and elevated levels of uric acid.

My rheumatologist prescribed Methotrexate, an immunosuppressive drug. This medication has been used very successfully in the treatment of psoriasis and psoriatic arthritis. I take a low dose of this drug once weekly.

Methotrexate works by binding to and inhibiting an enzyme involved in the rapid growth of cells, thus slowing down the rate of skin cell growth. It was originally used in the treatment of cancer but was discovered in the 1950s to be effective in the treatment of psoriasis and was eventually approved for use in this condition in the 1970s.

Methotrexate is generally well tolerated in small doses but it does potentially have a number of side effects. For this reason, it is imperative that a patient on this drug follows the instructions of their physician very carefully. People taking this medication need to have regular blood tests to be sure that the body is processing the drug safely and not creating other problems, particularly in the liver.

I have been on Methotrexate now for around four months. During the first six weeks or so, I noticed little reduction in my pain levels although the skin condition did show signs of subsiding. However, in the past couple of months, my need for painkillers has definitely been reduced and the skin condition is almost gone.

My family laugh when they see how excited I get about having knuckles now instead of great patches of scales. I can even go barefoot without the embarrassment of the condition in the webbing of my toes. Some people used to think I had a severe case of tinea and would look at me as though doubting my hygiene habits.

The only downside I have is a feeling of tiredness and the fact that I am prone to infections. This is caused by a reduction in my white blood cell count. However, under careful monitoring by my doctor, these symptoms are reduced by antibiotics when needed and a good multivitamin.

Despite these downsides, my life has greatly improved and continues to do so. I am kept aware of all the possible side effects and regular blood testing keeps my physician and my rheumatologist informed about any concerns. Hopefully, I will be able to live a far better quality of life in the future.
